Welcome to Naturally Yoga.  Please read the following information to ensure you and your child's safety and well-being.  The following states the guidelines upheld by Naturally Yoga to provide the best service for all participants.  Please feel free to ask any questions before agreeing to the below policies.

I have disclosed any information that will support my child at Naturally Yoga.  I believe that my child can participate in Naturally Yoga without any limitations.  As with any physical activity, checking with a physician is always recommended.  I will express any limitations to Naturally Yoga for the benefit of all.

Make-up Class and closing due to weather

Each session a child may use two make up classes.  This cannot be carried over to a new session nor is there reimbursement for missed classes.  In the event of dangerous weather, for the safety of all, Naturally Yoga reserves the right to close.  A message will be posted on Naturally Yoga's telephone answering machine.

Health Agreement

Every child is welcome in class in good health without any question of illness.

Returned Checks

There will be a $20.00 service charge for returned checks.

Liability Waiver

All participants in Naturally Yoga classes or workshops must sign a liability waver prior to participating in class.

Waiver of Liability and Release

Each participant of Naturally Yoga LLC assumes fully all ordinary risks when he/she participates in a session and agrees not to hold Naturally Yoga LLC or any teacher associated with Naturally Yoga LLC liable for injury or damage which may occur to him/her as a result of participation in a session or related activities of Naturally Yoga LLC.  Each participant of Naturally Yoga LLC takes full responsibility for personal property and agrees not to hold Naturally Yoga LLC or any teacher associated with Naturally Yoga LLC responsible for any loss that may occur during any scheduled session.  By signing this Release and Waiver of Liability participants are fully aware that their participation in Naturally Yoga LLC is at their own risk and agree that Naturally Yoga LLC or any teacher associated with Naturally Yoga LLC shall not be held liable for any injury, loss, or property damage resulting from active or passive negligence on the part of Naturally Yoga LLC.  As with any physical activity, checking with a family physician is always recommended.

If filing this registration online check the "I Agree" and  click the Submit Button.  You will be asked to sign this form in person when your child arrives for their first class.  If registering by mail, print and sign this form and mail it along with appropriate payment.

In addition to submitting this form payment must be made by either paying though our online storefront or by mailing a copy of this form with a check for the total amount.

BY MY SIGNATURE I INDICATE THAT I HAVE READ FULLY AND UNDERSTAND THIS AGREEMENT AND SIGN BELOW ON MY OWN FREE WILL.  SIGNATURES OF PARENT/GUARDIAN ARE ON BEHALF OF A MINOR.
